Optum Home & Community Care, part of Optum, is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members.

This field-based role emphasizes education, gap closure, and collaboration with primary care providers, with flexible scheduling and no weekends or holidays. Must hold NP/PA certification and an active state license, plus a valid driver’s license and reliable transportation.

Qualifications

• Active clinical certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for NPs or NCCPA for PAs.
• Active, unrestricted licensure in practicing state; ability to obtain by start date.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Provide education, identify care gaps, and collaborate with primary care providers.
