Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage and other members. The role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible scheduling (8am–8pm, 7 days), no on-call, weekends, or holidays. Sign-on bonus of $10,000 for external candidates accompanies a competitive compensation package and benefits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date).
• Active and unrestricted driver's license; access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments in different settings.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to address gaps in care.
• Educate members on disease processes, medications, and adherence.
• Recognize urgent situations and intervene appropriately.
