Job summary

UnitedHealth Group's Optum HouseCalls team is hiring an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members, focusing on education, gap closure, and collaboration with primary care providers.

This field-based role promotes patient-centered care in the home, with flexible scheduling between 8am–8pm across a 7-day week, no on-call, weekends, or holidays, and includes a $20,000 sign-on bonus for external candidates.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or AGPC.
• PAs: Current NCCPA certification and state licensure.
• Active, unrestricted licensure in practicing state and valid driver's license.

Responsibilities

• Conduct comprehensive in-home assessments including history, meds, vitals, exam.
• Identify and document diagnoses for care management and planning.
• Communicate findings to PCPs to close care gaps.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, meds, and adherence.
• Address social determinants of health and provide referrals.
