UnitedHealth Group’s Optum HouseCalls team seeks an Advanced Practice Clinician (NP/PA) to perform in-home Medicare assessments and coordinate care across settings. This field-based role emphasizes education, gap closure, and collaboration with primary care.

Qualifications

• One or more years of clinical experience in family, geriatric, or home health settings.
• Ability to educate patients on diseases, medications, and adherence.
• Strong collaboration with primary care providers to close gaps in care.

Responsibilities

• Conduct comprehensive in-home assessments including history, meds reconciliation, vitals and exams.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications and compliance.
• Address social determinants of health and provide referrals as needed.
