Job summary

UnitedHealth Group seeks an Optum HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes.

You will deliver high-quality care in patients’ homes with a balanced schedule, avoiding on-call, weekends, and holidays. Eligible NPs/PAs must hold current certifications and licensure in their practicing state.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted state licensure without disciplinary actions.
• Active and unrestricted driver's license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If independent practice is allowed, secure approval within the required timelines.

Responsibilities

• Conduct in-home comprehensive assessments including history, vitals, and exams.
• Document diagnoses for care management and planning.
• Report findings to PCPs to address care gaps and coordinate care.
• Identify urgent situations and intervene appropriately.
• Educate patients on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
