Job summary

Optum HouseCalls seeks an Advanced Practice Clinician (NP) to conduct in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with primary care providers.

You will perform comprehensive assessments, document care plans, and help patients navigate care. Flexible scheduling supports work-life balance and professional growth.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in practicing state by start date.
• Driver's license and reliable transportation.
• Ability to navigate varied home environments and perform assessments.
• Eligibility to practice independently where allowed; must apply within deadlines.

Responsibilities

• Perform comprehensive in-home assessments including history, med reconciliation, vitals, and exam.
• Document diagnoses for care management and plan.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
