Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling with no on-call, weekends, or holidays, and a competitive compensation package commensurate with experience.

Qualifications

• ANCC or AANP certification in Family, Adult, Geria tric, or Adult-Gerontology Primary Care.
• NCCPA certification and state licensure are required for PAs.
• Active, unrestricted licensure in the practicing state with no disciplinary actions.

Responsibilities

• Conduct in-home assessments, including medical history, meds reconciliation, vitals, and physical examination.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Identify urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
