Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You will conduct annual in-home assessments, document findings, and coordinate care to close gaps while supporting overall health goals.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state or ability to obtain by start date.
• Active and unrestricted driver’s license, plus reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments, including history, meds, vitals, and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
