Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and coordination with primary care providers.

The position offers a flexible schedule (8am–8pm, 7 days), with no on-call, weekends, or holidays required, and a $20,000 sign-on bonus or $40,000 student loan repayment option for eligible new hires.

Qualifications

• Active clinical license in the practicing state.
• Certification as NP or PA as applicable.
• Experience with elderly/Medicare populations preferred.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other members.
• Review medical history, medications, vitals, and perform exams.
• Educate patients and coordinate with PCPs to close care gaps.
