Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification and PA state licensure.
• Active, unrestricted state licensure to practice with no disciplinary actions.
• Access to reliable transportation for home visits.

Responsibilities

• Perform comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses and develop care plans in collaboration with PCPs.
• Educate members on disease processes, medications, and adherence.
• Identify urgent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
