Job summary

Optum Home & Community Care, part of the Optum family, delivers in-home health services including Medicare Advantage assessments. As a HouseCalls Advanced Practice Clinician (NP or PA), you will perform comprehensive in-home evaluations to improve outcomes and coordinate with primary care.

The role emphasizes flexible scheduling and career growth within an interdisciplinary team. Strong candidates will have NP or PA certification, active state licensure, and the ability to travel to patient

Qualifications

• Active NP or PA certification and state licensure
• Reliable transportation to perform home visits
• Ability to navigate varied home environments and perform assessments
• Experience with Medicare Advantage or home health settings is a plus

Responsibilities

• Conduct comprehensive in-home assessments including history, med reconciliation, vitals and exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals as needed
