Matrix Medical Network offers in-home comprehensive health assessments by Nurse Practitioners across the United States. Assessments last up to one hour and cover medical history, medications, and social factors to identify care gaps. Strong emphasis on patient education and coordination with PCPs.

The role features flexible scheduling, potential FT opportunities, and support for licensure and continuing education. Competitive pay is offered based on visit volume and geographic location.

Formación

• Master’s Degree or commensurate NP experience with licensure.
• Current NP licensure and prescriptive authority as needed.
• Board certification in Adult, Family or Gerontology NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply!
• Valid state driver’s license and automobile insurance for residence state.
• Strong computer skills and familiarity with health-record software.
• Excellent verbal and written communication skills.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education and referrals.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
