Matrix Medical Network offers in-home comprehensive health assessments conducted by Advanced Practice Providers. Assessments cover medical history, current health, medication adherence, and social factors to identify care gaps and improve outcomes.

Providers perform tests as needed and collaborate with a multidisciplinary team to craft tailored care plans. Flexible scheduling and field-based work support aging and at-risk populations across the United States.

Competenze

• Master’s degree or commensurate experience in nursing practice.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family, or Emergency NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcomed.

Mansioni

• Perform adult/geriatric health assessments including history, diagnoses, and education.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers for coordination of care.
• Provide patient education and follow-up in coordination with PCP.
• Deliver care in member's residence, including homes or skilled nursing facilities.
