Job summary

Matrix Medical Network is hiring nurse practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to identify care gaps. You will deliver patient education and coordinate with physicians and care teams.

The role offers flexible scheduling as home visits occur across various locations, including homes and skilled nursing facilities, with opportunities for full-time advancement.

Qualifications

• Master’s Degree or commensurate NP experience and licensure.
• Current NP licensure with prescribing authority or ability to obtain.
• Board certification in Adult, Family or Gerontology specialties preferred.
• Current BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment plans, education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCPs for patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facilities).

Skills

NP licensure in state of practice

Board certification (ANCC/AANP/etc)

BLS/ACLS certification

1 year NP experience preferred

Valid driver's license
