Job summary

Matrix Medical Network delivers comprehensive in-home health assessments to address gaps in care. Providers conduct geriatric assessments, review medical histories, and collaborate with physicians, case managers, and family members to support patient health at home or in SNFs.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Current NP licensure with prescriptive authority or ability to obtain.
• Board certification in Adult, Gerontology, Family, or Emergency care.
• Current BLS/ACLS certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and automobile insurance.
• Strong computer skills and familiarity with health record software.

Responsibilities

• Perform adult/geriatric comprehensive health assessments in home settings.
• Review medical history, diagnoses, treatments, and medication adherence.
• Coordinate with PCPs, case managers, and other care team members.
• Order and administer point-of-care and lab-based testing when needed.
• Document findings clearly using Matrix guidelines and protocols.
