Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform in-home comprehensive health assessments across the United States. The role involves reviewing medical history, medications, and social factors to identify care gaps and improve outcomes.

Responsibilities include conducting geriatric evaluations, ordering tests as needed, and collaborating with PCPs and care teams. Roles are full-time with sign-on opportunities and extensive support.

Qualifications

• Must hold NP licensure in state of practice.
• Master’s Degree in Nursing or equivalent.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance as required.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments including medical history and health education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide services at members’ residences, including homes and SNFs.
• Document findings clearly according to Matrix guidelines.
