Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform in-home comprehensive health assessments. You will review medical history, current medications, and social determinants to identify care gaps and tailor care plans.

This in-home role offers flexible scheduling, competitive pay, mileage reimbursement, and licensure support. Work with a multidisciplinary team in members' homes or SNFs across the United States.

Qualifications

• Master’s degree or commensurate NP experience and licensure.
• Current NP licensure with prescriptive authority.
• AANP/AACN/ANCC board certification in adult, gerontology, family, or emergency.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto coverage.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Assess adult/geriatric patients: medical history, diagnoses, education, referrals and follow-up.
• Order and perform point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Coordinate with PCP on education and follow-up care.
• Provide services at the member's home or skilled nursing facility.
