Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive health assessments in members' homes across the United States. You will review health history, medications, and social factors to identify care gaps and improve outcomes.

The role involves collaboration with PCPs and care teams, conducting required testing, and providing education during follow-ups. Partnerships with homes or SNFs enable flexible scheduling and ongoing professional development.

Qualifications

• Master’s degree in nursing or equivalent clinical experience.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, AGNP, FNP, or related field.
• BLS/ACLS certification available and current.
• 1 year NP experience preferred; new grads encouraged.
• Valid state driver’s license and auto insurance coverage.

Responsibilities

• Perform adult/geriatric health assessments including history and exams.
• Order and administer point-of-care testing and labs as needed.
• Collaborate with PCPs, case managers, social workers and families.
• Provide patient education and follow-up in coordination with care team.
• Deliver services at members’ homes or skilled nursing facilities.
