Job summary

Matrix Medical Network is seeking a Nurse Practitioner to conduct comprehensive home-based assessments across the nation. You will review medical history, medications, and living conditions to identify care gaps and improve outcomes.

Responsibilities include performing geriatric assessments, coordinating with physicians and care teams, and delivering care in patients’ homes or SNFs. Willingness to travel extensively and possess NP licensure and prescriptive authority is required.

Qualifications

Master’s Degree or commensurate experience and NP licensure.
Current NP licensure including prescriptive authority.
Board certification in Adult, AGPCNP, AGACNP, Family, Emergency, or Acute Care.
Current BLS, ACLS, or CPR certification.
At least 1 year NP experience.
Willingness to travel up to 80–90% including overnight travel.
Valid state driver’s license.
Auto insurance coverage for the state of residence.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.

Responsibilities

Conduct adult/geriatric assessments with medical history, diagnoses, treatment, and documentation per Matrix guidelines.
Administer and order point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary staff as appropriate.
Coordinate with PCP on patient education and follow-up as necessary.
Provide services at the member’s residence, which may be in homes or Skilled Nursing Facilities.
