Job summary

Matrix Medical Network delivers in-home comprehensive health assessments across the nation, enabling clinicians to provide personalized care at a member's home or skilled facilities. Practitioners perform adult/geriatric assessments, order tests, and collaborate with care teams to close care gaps.

The role offers flexible scheduling, competitive pay, mileage reimbursement, and ongoing education support. Qualified candidates hold a Master’s degree with NP licensure and prescriptive authority,

Qualifications

• Master’s Degree or commensurate NP experience with licensure.
• Current NP licensure and prescriptive authority.
• Board certification in applicable NP specialty.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance.
• Strong computer skills and EMR familiarity.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments with documentation.
• Order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
