Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments, reviewing medical history, medications, adherence, and living environment to close care gaps and improve outcomes.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP, AACN, or ANCC board certification in applicable NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
