Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or facilities. You will review medical histories, medications, and social factors to identify care gaps and coordinate with physicians and care teams.

The role offers flexible scheduling and the opportunity to impact patient outcomes while working within a supportive network. Training and ongoing education support are provided, along with a possible transition to full-time

Formación

• Master’s degree in NP with current licensure.
• Current NP licensure in state of practice.
• Board certification by ANCC/AANP or equivalent.
• BLS/ACLS certification.
• 1 year NP experience preferred.
• Valid state driver’s license.
• Experience with EMR systems and computer skills.

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and care teams.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence or in facilities as required.
