Zusammenfassung

Matrix Medical Network seeks licensed Nurse Practitioners to perform Comprehensive Health Assessments (CHA) at members’ homes or SNFs. Each visit lasts up to one hour and may close care gaps through in-home health review and documentation per Matrix protocols.

The role offers flexible scheduling, competitive pay by assessments, mileage reimbursement, 401(k), onboarding and continuing education support, and licensure assistance as needed.

Qualifikationen

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care NP specialties.
• BLS, ACLS, or CPR certification.
• 1 year NP experience preferred.

Aufgaben

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, family members and other staff as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).

Ausbildung

Master’s Degree or commensurate NP licensure

Current NP licensure in practice with prescriptive authority

Board certification: AANP/AACN/ANCC in NP specialties

BLS/ACLS/CPR certification

1 year NP experience preferred
