Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments (CHA) for adults, including medical history review, diagnoses, and education. Assessments occur at the member’s residence, with collaboration among physicians, case managers, and caregivers to identify care gaps.

Qualifications

• Master’s Degree in Nursing or commensurate experience.
• Current NP licensure in state of practice with prescription authority or the ability to obtain.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
• Administer and order point of care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities.
