Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments (CHA). Assessments review health history, adherence, and social factors to identify gaps in care and improve outcomes.

Providers work with physicians, case managers, and families, delivering care in patients’ homes or skilled nursing facilities. Successful candidates hold NP licensure in their state, board certification, and current BLS/ACLS; new grads are welcome.

Qualifications

• NP licensure and board certification required (Adult/Family/GERI/ACUTE care specialties).
• Minimum BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license and auto liability insurance for home-based visits.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment and education.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers and caregivers.
• Provide services at members’ residences, including homes or SNFs.
• Document findings clearly and follow Matrix guidelines and protocols.
