Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments (CHA) across the United States. Evaluations include medical history, care gaps, and appropriate referrals, conducted in patients' homes or SNFs.

The role requires NP licensure, board certification, and prescriptive authority where applicable. Strong communication and computer skills are essential for coordinating with care teams and documenting findings.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct comprehensive adult/geriatric assessments including history, diagnosis and treatment planning, education, referrals, and follow-up per Matrix guidelines.
• Order and administer point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family/caregivers.
• Partner with PCP on patient education and follow-up.
• Provide services at the member’s residence, including homes and Skilled Nursing Facilities.
