Matrix Medical Network is expanding its in-home clinical team across the United States, offering a competitive pay structure and flexible scheduling. You will conduct comprehensive health assessments in members' homes or SNFs, review medical histories, medications, and risk factors, and coordinate with physicians and care teams to close care gaps.

Ideal candidates hold NP licensure with prescriptive authority, BLS/ACLS, and at least 1 year of NP experience.

Formación

• Master's Degree or commensurate NP experience.
• Current NP licensure in state with prescriptive authority.
• BLS/ACLS or CPR certification.
• 1 year NP experience.

Responsabilidades

• Perform adult/geriatric health assessments in home or facility settings.
• Administer point-of-care tests and coordinate lab work as needed.
• Collaborate with PCPs, case managers, and family members.
• Provide education and follow-up to patients and caregivers.
