Job summary

Matrix Medical Network seeks compassionate NP providers to perform Comprehensive Health Assessments in patients' homes across the state. Assessments last about one hour and cover medical history, meds, adherence, and risk factors, with follow-up and documentation per Matrix protocols.

We offer flexible scheduling (weekends and weekday slots), competitive pay based on 20–100 visits monthly, and support for licensure and ongoing education. A valid NP license and driving safety are required.

Qualifications

Master’s degree or commensurate experience with NP licensure
Current NP licensure to practice and prescriptive authority as needed
AANP, AACN, or ANCC board certification in Adult/Family/Gerontology specialties
BLS, ACLS, or CPR certification
1 year NP experience preferred; new grads welcome
Valid state driver’s license and auto insurance as required
Strong computer skills and proficiency with EHR/health IT systems
Excellent verbal and written communication skills

Responsibilities

Conduct adult/geriatric health assessments including history, diagnoses, and follow-up
Administer and order point-of-care and lab testing as needed
Collaborate with PCPs, case managers, social workers and family care partners
Provide services at members' homes or SNFs as appropriate
Document findings clearly and in accordance with Matrix guidelines
