Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in members' homes across participating states. Each visit lasts up to one hour and covers medical history, medications, and risk factors, with findings documented per Matrix guidelines.

Providers administer point-of-care tests as needed, coordinate with PCPs and care teams, and may travel beyond their home location.

Formación

• Must hold an NP license and board certification in a recognized NP specialty.
• Master’s degree or commensurate clinical experience required.
• Current BLS/ACLS certification is expected.

Responsabilidades

• Conduct adult/geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
• Administer and order point of care and lab-based testing as needed.
• Work with physicians, case managers, social workers, family members, and ancillary medical personnel as appropriate.
• Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary.
• Provide services at the member’s residence which can be in their homes or Skilled Nursing Facilities.
