Job summary

Matrix Medical Network is seeking a Nurse Practitioner to conduct in-home comprehensive health assessments for at-risk adults across the United States. Each visit can last up to one hour and focuses on health history, medications, adherence, and home environment to identify care gaps.

Providers will order tests as needed, collaborate with PCPs and case managers, and deliver clear documentation. Flexible scheduling and opportunities for full-time roles with onboarding support are available.

Qualifications

• Master’s Degree or NP-equivalent experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• BLS, ACLS or CPR certification.

Responsibilities

• Perform adult/geriatric health assessments and document findings.
• Order and interpret point-of-care and lab tests as needed.
• Collaborate with physicians, case managers and families to coordinate care.
• Provide in-home health assessments at member residences.
