Job summary

Matrix Medical Network seeks a Nurse Practitioner to conduct in-home comprehensive health assessments, reviewing health history, medications, and risks during visits in members' homes or SNFs. You will order tests, coordinate with PCPs and care teams, and provide education and follow-up as needed.

Flexibility in scheduling is offered, with opportunities for a path to full-time roles. Prior NP experience and licensure in the state of practice are required or encouraged to obtain licensure as

Qualifications

• Must hold current NP licensure in the practice state.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties preferred.
• BLS, ACLS or CPR certification required.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license and auto insurance coverage for state of residence.

Responsibilities

• Perform adult/geriatric health assessments at member homes or facilities.
• Review medical history, medications, and risk factors; document per Matrix guidelines.
• Order point-of-care tests and lab work as needed.
• Coordinate with PCPs, case managers, social workers, and family caregivers.
• Provide patient education and follow-up in collaboration with healthcare providers.
