Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members across the United States. You will review medical history, medications, and functional status during visits that last up to an hour, delivering focused care and actionable documentation.

You will collaborate with physicians, case managers, and caregivers to identify care gaps, provide patient education, and coordinate referrals.

Qualifications

• Master’s Degree in Nursing and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult, Gerontology or Family.
• BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license and auto insurance for state of residence.
