Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other plans. You will work in a field-based role focused on education, gap closure, and collaboration with PCPs to improve health outcomes.

The role offers a flexible schedule with 8am-8pm coverage, no on-call, no weekends, and a $10,000 sign-on bonus. A comprehensive benefits package and growth opportunities are included.

Qualifications

• Active ANCC or AANP Family, Adult, Geriatric, or Adult-Gerontology Primary Care certification.
• Active, unrestricted licensure in the practicing state with no disciplinary actions.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including history, vitals, and exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps and continuity.
• Recognize urgent situations and intervene appropriately.
• Educate patients on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
