Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

The position offers flexible scheduling within 8am-8pm, 7 days a week, with no on-call, weekends, or holidays required, and includes a $10,000 sign-on bonus for external candidates and a comprehensive benefits

Qualifications

• Active certification as NP or PA (Family/Adult/Geriatric primary care)
• Louisiana licensure in good standing or ability to obtain by start date
• Access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, vitals, exam, and appropriate screenings
• Identify diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications, and compliance
• Address social determinants of health and provide referrals as needed
