Job summary

UnitedHealth Group's Optum HouseCalls program seeks an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

Flexible scheduling and a $10,000 sign-on bonus for external candidates are offered, with no on-call, weekends, or major holidays.

Qualifications

• Active and unrestricted licensure in the state you practice, without disciplinary actions
• Certification in Family, Adult, Gerontology Primary Care or NCCPA for PAs
• Ability to obtain licensing in the start state by start date if required

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medications, vitals, and exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify diagnoses for care management and plan treatment
• Communicate findings to PCPs to close care gaps
• Educate patients on diseases, medications, and adherence
