Job summary

Matrix Medical Network hires providers to conduct comprehensive in-home health assessments across the country. Each visit lasts up to one hour and evaluates health history, adherence, and social factors to close care gaps.

NP-led teams review data with physicians and care managers to improve outcomes. Providers deliver services at members' residences, including homes and Skilled Nursing Facilities, with support for licensure expansion and ongoing education.

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority if required.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care.
• Valid BLS/ACLS or CPR certification; driving license required.

Responsibilities

• Conduct adult/geriatric assessments including medical history and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for education and follow-up.
• Provide in-home or SNF-based health assessments.
