Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments across diverse populations, including geriatric patients. You will review medical histories, conduct exams, and coordinate with physicians to close care gaps while delivering care in patients' homes.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Auto insurance coverage for the state of residence.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, follow-up, and documentation.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and family members as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).
