Job summary

Matrix Medical Network is seeking licensed nurse practitioners to perform comprehensive in-home health assessments. Duties include reviewing medical history, administering tests, and coordinating with physicians to close care gaps. Work is home-based or at SNFs with flexible scheduling based on availability.

Compensation is per assessment with potential for full-time opportunities and a sign-on bonus in some locations. Onboarding, mentoring, and licensure support are provided to help you grow.

Qualifications

• Must have NP licensure in state of practice.
• Current BLS/ACLS or CPR certification.
• AANP, AACN, or ANCC board certification in applicable NP specialty.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance as required by residence.
• Strong computer skills and familiarity with health records software.

Responsibilities

• Conduct adult/geriatric health assessments including medical history and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Provide patient education and follow-up in home or SNF settings.
• Coordinate with PCP on follow-up care and referrals.
