Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in members' homes across California. Each visit lasts about an hour and includes review of medical history, medications, and social factors to identify health risks and care gaps.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP license with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Acute Care.
• BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license for practice state.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab-based testing.
• Coordinate with physicians, case managers, social workers, and other caregivers.
• Collaborate with PCPs on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).

Education

Master’s Degree or commensurate experience and NP licensure

Current NP licensure with prescriptive authority
