Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes nationwide. Each visit lasts up to an hour and reviews medical history, medications, adherence, and risks to identify care gaps and support better outcomes.

You will collaborate with physicians and care teams, conduct necessary tests, and document clearly. Experience with EMR software is valued; licensure and continuing education support are provided.

Qualifications

• Must hold NP licensure in the state of practice with prescriptive authority if needed.
• Board certification in appropriate NP specialty (adult, geriatrics, family, or acute care).
• Current BLS/ACLS or CPR certification is required.
• 1 year NP experience preferred; new grads may apply.
• Valid driver’s license and auto insurance coverage for state of residence.
• Proficiency with electronic health/medical record software.

Responsibilities

• Perform Comprehensive Health Assessments in patients’ homes or SNFs.
• Review medical history, diagnoses, medications, and adherence; document per Matrix guidelines.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, and care teams for patient education and follow-up.
• Provide clear, actionable documentation and coordinate care gaps to improve outcomes.
