Job summary

Matrix Medical Network is hiring NP providers to perform comprehensive in-home assessments for seniors and at-risk individuals. You will review medical history, medications, and social factors to identify care gaps and improve outcomes.

Travel to member homes or SNFs is expected, with coordination among physicians, case managers, and family members. Role requires NP licensure, board certification, and BLS/ACLS; strong communication and documentation skills are essential.

Qualifications

• Master’s degree in nursing and NP licensure in state of practice.
• Board certification in a primary care or gerontology specialty.
• Current BLS/ACLS/CPR certification and eligibility for prescriptive authority.

Responsibilities

• Conduct comprehensive health assessments in the member's home or SNF.
• Review medical history, medications, and social determinants of health.
• Coordinate with PCPs, case managers, and caregivers for follow-up.
• Administer point-of-care tests and order labs as needed.
• Document findings per Matrix guidelines and ensure clear reporting.
