Job summary

Matrix Medical Network is hiring advanced practice nurses to perform comprehensive in-home health assessments. These clinicians review medical history, medications, and social factors to improve care quality and close care gaps.

The role emphasizes patient education and coordinated follow-up in home or SNF settings. Qualified NPs holding state licensure, board certification, and CPR/BLS are encouraged to apply.

Qualifications

• Master’s degree or commensurate NP experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Family, or Gerontology specialties.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license and auto insurance coverage.

Responsibilities

• Conduct adult/geriatric health assessments and document findings per Matrix guidelines.
• Administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide in-home/Skilled Nursing Facility visits with education and follow-up.
• Maintain clear documentation and coordinate with PCP for follow-up.
