Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing the member’s medical history, adherence, and living context to identify care gaps. Assessments occur in patients' homes or SNFs, with coordination to PCP for education and follow-up.

The role emphasizes flexible scheduling, nationwide opportunities, and potential for full-time positions with a per-assessment framework and ongoing professional support.

Formación

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification in one of the listed specialties
• Current BLS, ACLS or CPR certification

Responsabilidades

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, follow-up and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and caregivers as appropriate
• Provide services at the member’s residence (home or SNF) and coordinate with PCP for education and follow-up
