Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments across our national network. You will review medical history, medications, and social factors to identify care gaps and coordinate with physicians and care teams.

Qualifications

• Must have NP licensure in state of practice
• Board certification in relevant NP specialty
• BLS/ACLS certification or CPR
• Valid driver's license and auto insurance
• Strong communication and computer skills

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans
• Administer and order point-of-care and lab-based testing
• Collaborate with physicians, case managers, social workers and caregivers
• Provide in-home health assessments at patient residences or SNFs
• Coordinate with PCP for education and follow-up
