Job summary

UnitedHealth Group's Optum Home & Community Care team seeks a HouseCalls Advanced Practice Clinician (NP) to perform in-home health assessments for Medicare Advantage and other plans. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

You’ll work under a flexible schedule (20–40 hours/week) with a $10,000 sign-on bonus, no on-call, weekends, or holidays. Competitive benefits provided.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geraitric, or Adult-Gerontology Primary Care.
• Active, unrestricted license in the practicing state by start date.

Responsibilities

• Conduct comprehensive in-home assessments including history, meds reconciliation, vitals and exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications and adherence.
• Address social determinants of health and arrange referrals as needed.
