UnitedHealth Group's Optum HouseCalls is seeking a Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active NCCPA certification and state licensure for PAs.
• Active and unrestricted licensure in the practicing state without disciplinary actions.
• Driver's license and reliable transportation for home visits.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Educate members on disease processes, medications and compliance.
• Collaborate with PCPs to address gaps in care and plan management.
