Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes and keep patients connected to care.

You'll conduct comprehensive assessments, document diagnoses, and address social determinants of health, with flexible scheduling (8am-8pm) and no weekends or holidays.

Qualifications

• Active ANCC or AANP Family/Adult/Geriatric/Adult-Gerontology PCP certification.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted state licensure and ability to obtain start date.
• Active driver's license and access to reliable transportation.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, medication reconciliation, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
