Job summary

Optum Home & Community Care seeks a HouseCalls Advanced Practice Clinician (NP/PA) to perform comprehensive in‑home health assessments for Medicare Advantage and other plan members.

You will document diagnoses, collaborate with PCPs to close care gaps, and educate patients on diseases, medications, and adherence. The role offers flexible scheduling and strong support for professional growth.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Current NCCPA certification and state licensure.
• Active, unrestricted licensure in the practicing state (or ability to obtain by start date).
• Active and unrestricted driver's license with reliable transportation for home visits.

Responsibilities

• Conduct in‑home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications and adherence.
• Address social determinants of health and provide referrals.

Education

ANCC or AANP Family/Adult/Geriatric/Adult‑Gerontology Primary Care certification

NCCPA certification and state licensure

Active unrestricted state licensure

Driver's license with reliable transportation for home visits
