Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls program to deliver in-home health assessments to Medicare Advantage and other plan members. This role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

The Advanced Practice Clinician (NP or PA) will perform comprehensive in-home assessments, document diagnoses, and support patients through care transitions, with flexible scheduling and no weekends.

Formación

• Must have active ANCC or AANP certification for NP track or NCCPA certification and state licensure for PA track.
• Unrestricted licensure in practicing state (or ability to obtain by start date) without disciplinary actions.
• Active and unrestricted driver’s license with reliable transportation for home visits.

Responsabilidades

• Conduct in-home assessments including medical history review, vitals, and physical examination.
• Document diagnoses for care management and treatment planning and communicate findings to PCPs.
• Educate patients on disease processes, medications and adherence; address social determinants of health and provide referrals.
