Job summary

Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls team and seeking an Advanced Practice Clinician (NP/PA) for in-home Medicare Advantage assessments. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes for patients living at home.

The successful candidate will conduct comprehensive in-home evaluations, document diagnoses, and coordinate care with PCPs, while navigating diverse home

Qualifications

• NPs: ANCC or AANP Family/Adult/Geriatric Primary Care certification.
• PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure in the state of practice.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history, vitals, and exams.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Educate members on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
